I'm here to report that this is still an issue. I guess I got myself a ghost bow.

With Mastermind Thug pets, if you have them equipped with both upgrades, when you change zones it reverts their costume to as though you only have one of the upgrades. They have equipped both upgrade's powers, however it just doesn't show in their costumes (e.g. Enforcers only have leather jackets instead of trench coats). If you give your pets either of the upgrades after you zone it will show them with the correct costumes again.

In the regular Sewer Network on the map the exit marker for Kings Row is labeled "Port Oakes". EDIT: This only happens when playing as a Rogue. When playing as a hero it's labeled correctly.

I also have the issue. Officer Blackerby Everlasting Character created on 2019-11-18 Earned Passport on 2021-02-07 Transferred from Torchbearer to Everlasting, but I do not remember when he was transferred
