On rare occasions during the final fight with Tyrant in his big form, when you burn the big form down, his regular sized version doesn't spawn. Making it impossible to complete the trial at that point and robbing Incarnate Salvage from players

Indeed. It is a fairly rare occasion, but does happen. Seems like it happens due to the speed at which he's defeated.

Tyrant is supposed to become untargettable and transform into his regular sized version at 10% health. If your league does enough damage that you take him all the way to 0 health before the untargettable status takes effect, he's dead and can't transform. Just try not to kill him too fast next time.
