Have had this happen several times lately, where the "Mission Complete" appears, but I had not completed all the Objectives. In the Most recent case it was the 2nd Mission for Mu'Vorkan, I still had 2 hostages to rescue and when I Defeated the last Leader, the Mission Completed. I searched areas of the Map I had not yet explored and found the 2 hostages, each being guarded by a Mob. 

On a side note: In the 1st Mission where you are to rescue Scirrocco and Ice Mistrel, I have come to believe that both of them are possessed by the Spirit of Leroy Jenkins. Scirrocco would run out of the room past an entire crowd of villians and aggro enemies in multiple mobs until he had an unmanageable Mob and they killed him, then I had to clean up the mess. Ice Mistrel would leave the Mob we were fighting and Snipe at targets in another Room. C'mon Leroy gimme a break, I thought Fussionette was Crazy, lol. I usually enjoy Pet Missions since because of when and where I play there are not a lot of team options, but that was irritating.

I've been noticing this kind of thing a lot ever since I came back to the game. I'm sure I remember needing to completely clear maps, but now it seems I only need to defeat the "leader" and the handful of mobs in his immediate vicinity. I usually still finish clearing them out, though, for XP and drops.

 

As for the OP's side note - yeah, the helper characters in some missions seem to have become undesirably aggressive lately. I no longer make any attempt to keep No Mind alive in that one Shining Stars mission, since he consistently charges right into the middle of the first room that's full of Praetorian clockwork and gets his ass kicked. And on the last three alignment missions I've had where Overdrive is present, she has aggro'd on groups in different rooms or on different levels from where I'm fighting, and got clobbered. This seems like new behavior - in the past she's always stuck close to me. Which reminds me of something that has always bugged me: why do these characters insist upon flying inside buildings? IMO, they should only fly if I fly. Otherwise, stay grounded.

 

In another Shining Stars mission, Grym (who is supposed to be the best-trained fighter in the group) keeps dying on the first fight because he insists on standing in the fire, but I think that has always been the case.
