Just noticed a new bug today and not sure if it has been reported yet. I just used 2 enhancement catalysts on different controller AT enhancements and when in the screen to combine the enhancement and the catalyst, the catalyst no longer showed up inside second circle. I attached a picture showing what it looks like.

Similar issue with trying to use a Catalyst on "Blaster Wrath: Recharge/Chance for Fire Damage"; except after hitting "Combine" the screen would just stay at the "Combining" text but never would proceed.  Clicking "Exit" would stop it, but it would consume the Catalyst without upgrading the enhancement.

Update to the above post.  Tried Catalyzing another Enhancement in the Blaster's Wrath set and it seemed to do fine.  Tried the "Recharge/Chance for Fire Damage" again and it still would not work correctly.

Make sure you do not have another window opened up still . like your salvage window for base salvage bins. My example was I had 6 catalysts stored in my base salvage bin. I opened the window and took 6 catalysts out and then clicked on manage and tried to convert without closing the salvage windows. I had the same experience as you. I could select the combine window. Select the catalyst and select combine but it would never combine. I had to select cancel also. and saw that I only had 5 catalysts left. However, when I checked the salvage bin I found the catalyst was back in the bin. So, if that is what you did then you shouldn't have lost anything. just need to make sure to close the windows after taking the catalysts from the bins before trying to catalyze them.

I've also experienced this bug, most recently failing to convert an Aegis Psi/Status to a superior version.

I have experienced this, but it seems to be exclusive to when I've tried to catalyze things inside the base. It doesn't do the same thing outside of a base.  So now I step outside if I want to catalyze something. Or respec. I've heard it's a bad idea to respec in a base.

Having experienced this issue myself, a warning tip and/or alert about catalyst use while inside a base would be an extremely helpful QoL matter.
