Hero/Villain not showing up during bank missions

For the last few weeks the Hero or the Villain will not show up during bank missions. This does not happen every time, but is happening more frequently over the last few weeks (especially the hero not showing up). The horde of longbow agents shows up, but the hero never shows. It happened 3 out of 4 times yesterday and once today (1 out of 1).

I am getting this as well.  3 times today no show on the PI Mayhem mission
