Trying to see if there is anything I can do to improve the build or any mistakes I am making

You do realize you can take powers earlier than you are correct? See Fulcrum Shift. That can be taken at 30 now. 

 

Similar, Tough and Weave are really early. 

 

Transfusion needs Acc. Grab one of the Accurate Healing sets. 

 

I would grab Siphon Power earlier and leave Maneuvers for later. 

 

Hasten could use another slot. 

 

Siphon Speed could use some more slotting.

 

More slotting for FS. You want some recharge in there as well.

 

ID is a far better place for the one off Resist IOs which offer +3% Def.

 

Speaking of which, something has to be switched on for your Def stats to be that insane. There has to be an Incarnate or some such thing flipped on to get those numbers. 

 

Since I can't directly load your build in, it is hard to say much more.

To add, your power selections at 12 and 41 aren't displaying unless I am missing something. I would just upload the file to the forum.
