Are combinations like Battle Axe / Shield at a disadvantage from the get-go or is there an approach to making something like this work? I don't have experience, but I'm thinking that these sets become a race to put the enemy down as opposed to being able to sustain a longish fight.

Are they to be avoided? Do you load up on healing inspirations? Are there "chance to heal" procs that are used?

I use inspirs to fill in whatever the build might be lacking so needing greens on hand isn't that big of a deal. Oranges are helpful as well. I do use Chance to Heal procs where I can, see Stamina. The thing is, Def based sets are going to by a default have some if not max amount of DDR so getting hit isn't really that big of an issue. More so if your primary offers mitigation in stuns, KD, etc. I had a Shield/mace Tanker and dying really wasn't an issue. SR can be insanely sturdy once it comes together despite being a 'one-trick pony.' SR fears +ToHit more than anything. In all honestly, dealing with end usage would be the bigger thing to deal with for a Axe/shield Brute than being vertical.

Not sure what SR is in this context, super reflexes? What is "SR Fears"?

 

Edit: Figured it out. SR = Super Reflexes. You're using it as an example because it doesn't have heals. "SR Fears" isn't a power, you're saying that a Brute with Super Reflexes fears enemies with +ToHit buffs.

Super reflexes, like shield, have really good defense debuff resistance (DDR).  This means debuffs that lower defense is resisted.  So the next thing that allows baddies to get through your defenses is baddies that have +ToHit.  For a mature shield or super reflexes toon, ToHit is the bigger problem than healing.
