Incarnate/Temporary buff icons not displaying

Recently incarnate alpha/hybrid(passive)/interface buffs stopped appearing as icons beneath my xp bar with the rest of my toggles. Also missing is any secondary mutation buff and my "countdown to frenzy" for having recently changed back to villain. What does still appear are toggles/hasten/ and incarnate hybrid(toggle)/destiny.

 

I first noticed this after a death, but do not know for sure if it changed when I died or before this. I have reloaded the game and looked for any associated options but can find nothing that might cause this. I have also tried logging in from another PC and am getting the same behavior. This only happens on the one character.

Right-click on any of the powers in your buff bar and unselect "hide auto powers."
