Every so often in a mission you will get npc civilians running around scared, and I was an option in AE to get that added in or a way to cheat it in. Creating a "Kidnapp a Scientist" mission and think it would be better visual storytelling if there were other scientists working in the lab that you are breaking into.

You can create friendly NPC patrols, but they won’t be scampering. NPCs as rescues with the captors set to empty and the NPC trembling, then running off when you rescue them might work, but it’s limited. Unfortunately, there just really isn’t a way to achieve that effect you see in in-game mishes.

Well that is a shame. I don't think some calm walking patrols of scientists will blend with the idea of a smash and grab of a person of interest. Maybe I can use non-essential quests of rescues being menaced by Arachnos soldiers trying to also find hte person but then it also breaks the story a bit since the players are being hired by arachnos. maybe a rival cell, or would that be too complicated to introduce in part 3 of a 5 part mission?

My advice is to design the main narrative as solid as possible (the “must do”). After that, worry about the other stuff (the “would be nice if…”).  There is, after all, nothing we can do but work with what we have.

Yeah, we've asked for this before (we've asked for tons of stuff many times, and we're still waiting for even a brief "sorry we couldn't do that because blah blah..."), it would be really nice.  I've got two arcs in production that could use it.
