Only thing I see to call out is no end reduction slotted on Tough and Weave.

 

I'm leveling this same combo, and I'll post a build once I do one in mids.

I'm lost with your slotting. Are you aiming for hp bonuses/toxic res/slow bonuses/cold res...thats where i got really confused. I was about to manually input it as the link won't open and theres no data chunk but i gave up when i saw the 3 slotting build bonuses. So i just throw in stuff at random too:
