For some reason I am not able to change my male villain to female with the facemaker person.  I am able to do this just fine on hero side.  Anybody have any ideas what is going on?

You're looking for the "surgeon" not the tailor. They're a different npc deeper in the clinic villain side. It's the "Facemaker Surgeon" Hero side I think it's the "Cosmetic Surgeon" but not sure on that.

Hero side it's always the one at the back of the Icon store on the right side / corner.

 

The tailor in Rikti War Zone also is a cosmetic surgeon.  I don't remember if the one in Pocket D is or not.

The D tailor - in the Tiki Lounge, on the top floor facing the door for those unaware of her location - is in fact a full-service tailor, yes.

So is every trainer. They're all 'surgeons'.

 

YOu can change your costume and gender at Miss Liberty, straight out of character creation, which means you don't even have to run across the street to get into the Vanguard base and the tailor/surgeon there. Pretty sure the same is true villain-side.

... since when? Height and Body exclusive to surgeons as far as I can tell and the UI is telling me.

The trainers are standard costume change people now. You can alter the looks of your costume and power sets, not gender/body type. You have to got a surgeon to do that.
