how do you create Hero One DNA Sample from Threads, etc.

I see you can create these items, Hero One DNA Sample, Dimensional Keystone, etc. from Incarnate Shards, but there is no interface I can see to create them from Threads. Where do I look or is this old information from 2012?,

Try scrolling down in your Incarnate window.  Thread options are below the shard options, IIRC.

You can't.  You can only get shard based ingredients from shards or from the appropriate endgame TF/raid/trial.  Shards (and shard ingredients) can convert to threads but not the reverse.

That's complex. I guess the wiki has that info, hopefully just a list of which TF gives which shard item.

You can’t create those particular components from threads, but the components that you can create from threads can be used to create alpha incarnate powers.

 

As Tafilr said, It’s a separate recipe hidden below the first alpha recipe. If you scroll down, you should find it.

 

Might save you some trouble, unless you feel like you’re committed to the shard recipes.

FWIW, shards and the shard components are ONLY good for alpha abilities.  Once you get your chosen alpha to t4 the shards are of no further use and you have nothing better to do with them than to convert them to threads, which remain useful until everything you want is at t4.
