Yes. They have tried to break into the us market a few times, with xox master or whatever that top down game was called, and some very small endeavours that i cant remember the names of offhand.

It is a great equalizer among the masses... Why can't new/additional content be built with other equalizing factors that do not adversely affect non incarnates, but still punch hard enough to hit incarnates?

This has its ups and downs, and I am mixed about how I feel about it. Maybe its good, maybe notsomuch. It depends on a lot of things, and honestly, sometimes players simply havent seen that content at all and ripping it up so they can explore safely what things they haven't seen works for me some days. Kind of like running certain arcs or whathaveyou that arent commonly done. Get the big guns out, help new to the arc/content players enjoy it without being slaughtered.

Whatever the cap is for hero merits. It used to be 9999, but with hero merits, you can convert 50 to 1 on the same char, and then back them out. So literally, 9999 is the cap for _regular_ merits, but hero merits at a 50 to 1 ratio have a different cap. Wouldn't be surprised if that was 9999 also.... but I dont have anyone with 500k merits to test it with.

Yep. But to target a little closer, there are people who never look at Mids/MRB. There are people who do, but casually slot, and if they never complete there build, MEH it works and smashes things and they are happy. And then there are people who dont market, have some sort of farmer that may or may not afk, but really just dont care that much beyond some inf here and there. For me, thats casuals. People that build out every build, twinked to max efficiency, definitely not a casual. Casuals (usually?) just dont care about 8/4. Feeling super matters more, and +1 or +2 to them with some random things they can smash around is good enough. The comment that all HC players are casuals, I don't see that. There are definitely some NOT casuals that do some strange things, like limit player participation in groups because it doesnt maximize x y or z thing they are looking for.

Goldside is full of awesome worldbuilding that was never in the game before. It is a shame it wasnt more popular. I think that people just didnt want to not-play-with-teammates, and goldside pretty much obliterates team playing with all of the side switching mixed up mish content.

You can do stupid damage with db with zero comboing. IIRC, the top damage from a scrapper db uses zero combos. Just lots and lots of rech.

sentinel was a fantastic choice. Go with a tank to support. Something with dark melee.

First link has rune of protection up, which i am pretty sure isn't perma, pretty positive there is downtime there. edit: they all do. I am not a fan of build posts with RoP on, since it doesnt reflect the steady-state of the build, but more of a peak operability.

Grinding, open up MRB, set it to offensive mode, and go from there. Build exclusively around offensive mode, because frankly, in end game content that is almost definitely where you will be all the time. The +tohit, damage, damage, and yet moar damage from offensive mode is crazytalk. The benefits from defensive and efficiency modes can just be slotted around. The pure damage of offensive there is no real way to compensate not using w/ set ios. Also, offensive gets you mad damage ups on fast snipes if you go that route, since fast snipe damage takes into account current tohit. With kismet and offensive, you are pretty close to slow speed snipe damage on a near instant cast snipe...
