Random heads-up; The Firebase Zulu mission door located at 3389.2 1092.2 -2354.1 outputs you inside a nearby tree which causes you to be unable to move.

Storm Palace, (-1014.3 2387.5 -50.2), at the Storm Palace itself.  There appear to be missing textures on the top and bottom of the two columns that show where you can get past the force field and into the mission entrance.

Difficult to submit a picture for this one.  I made a quick upload to Imgur to cover this one since it's something that would need to be visually demonstrated.

 

Echo: Rikti Crash Site, (2947.1 -7.0 -6231.0), near the Shielded badge.

Positioning the camera in just the right spot against the shield of the crashed mothership causes the texture on the surface of the mothership to freak out.  Flying horizontally along the western side of the ship, the texture freakout can be seen from multiple positions as long as the camera is positioned correctly.

Founder's Falls Safeguard Mission - PPD Jailbreak Exit Geometry Bug

 

While exiting PPD office, I fell through the map and into the below aether.  I repeated this several times and found that this only occurred when Hover was active.

I have seen this posted previously by Wavicle and others --- I wanted to emphasize how I was able to successfully retest no issue (Hover off) and issue (Hover on).  Feedback greatly appreciated.

This giant Council robot wreckage is missing textures on the inside of its command module.

EDIT: Another missing texture in the deep end of the swimming pool, [83.8 -230.5 781.1].  You can see the rocks behind the walls through it.  On a side note, i'm not sure how I got the camera under the water to begin with...

And, as I discovered experimentally with other gaps, it only takes a tiny gap to thread a Teleport or Translocation bullseye through to allow you to port to the other side.

Cap au Diable, the small plaza lowered area around the Base Portal.
The physical foorl is slightly above the graphical foorl.

Nope. 
Note the character feet vs the base of the phones and bench.
I'm walking.

Seems the entire area is sloped to the middle in the tiled area, and the characters ignore this.
Less noticeable at the edges.

In the mission 'Drive away the D.U.S.T. cell' from Nadia in First Ward I came across this potential glitch in the First Ward variant of one of the typical office map configurations.

 

Here I can see the mission boss through a gap in the walls. As I was playing a gravity controller I just wormholed him out through the gap and skipped all the troopers which lie between us if I had taken the path up the stairs... neat!

Or, with Mystic Flight and a handy nuke, Translocate through the gap and nuke the room.

Ruined office maps that use that room tile all have that quirk.  If you can teleport, you can skip the rest of the room.  Or you can be like me and drop Rain of Arrows through the gap and kill the group before rounding the corner.

Night Ward, [-1050.2 48.7 -2692.7].  May also be present in First Ward at the same coordinates.

 

This hatch in the DUST Compound has a large #2 on it.  That 2 doesn't seem to be attached to the hatch as it gets left behind when the hatch is opened for a mission or other reason.  Also the hatch itself clips into the building when opened.

There is a visible gap where two pieces of the outside of the Chantry connect.  This gap might be visible in other Chantry maps, and possibly the actual Chantry as well.

Right, not so much a geometry issue but I wasn't sure where else to post this.  It's a recent thing though, possibly since an update.

 

The effects of the paths in First Ward and Night Ward in these spots isn't working,
